TÓM TẮT NHỮNG KẾT LUẬN MỚI CỦA LUẬN ÁN

- Đặt vấn đề: Suy sinh dục nam có xu hướng gia tăng và phổ biến ở những người lớn tuổi. Khuynh hướng lựa chọn thảo dược hay bài thuốc cổ truyền để hỗ trợ điều trị suy sinh dục nam được quan tâm ngày càng nhiều.

- Đối tượng và phương pháp nghiên cứu: Viên nang KS đóng trong nang số 0 màu đỏ chứa 540 mg cao chiết dược liệu, bao gồm: lá Dâm dương hoắc (50%); Cửu thái tử - hạt Hẹ (20%); lá Đinh lăng (20%); lá Bạch quả (10%). Nghiên cứu thực nghiệm trên chuột, trước và sau can thiệp với cỡ mẫu nhỏ.

    - Kết quả:

1. Đánh giá tính an toàn: Sau 14 ngày, phác đồ kết hợp viên nang KS (1 viên/kg) với testosteron (1 mg/kg) không làm thay đổi nồng độ testosteron, protein toàn phần; trọng lượng cơ thể, túi tinh - tuyến tiền liệt, tinh hoàn, cơ nâng hậu môn của chuột nhắt đực bình thường so với lô chứng uống nước cất. Sau 60 ngày, phác đồ kết hợp (KS + testosteron) làm tăng nồng độ testosteron, protein toàn phần; tăng trọng lượng túi tinh - tuyến tiền liệt, tinh hoàn, cơ nâng hậu môn.

2. Mô hình cắt 2 tinh hoàn: Sau 15 ngày, phác đồ kết hợp viên nang KS (1 viên/kg) với testosteron (1 mg/kg và 2 mg/kg) thể hiện tác dụng tăng nồng độ testosteron, tăng protein toàn phần, tăng trọng lượng túi tinh - tuyến tiền liệt, cơ nâng hậu môn so với lô bệnh lý và tiệm cận với lô bình thường.

3. Mô hình natri valproat 500 mg/kg: Sau 35 ngày, phác đồ kết hợp viên nang KS (1 viên/kg và 2 viên/kg) với testosteron (1 mg/kg) làm tăng nồng độ testosteron; tăng trọng lượng tinh hoàn, túi tinh - tuyến tiền liệt, cơ nâng hậu môn so với lô bệnh lý. Phác đồ kết hợp có tác dụng bảo vệ mô tinh hoàn: cải thiện hình thái ống tinh; tăng số lượng tế bào Sertoli, tế bào Leydig; tăng số lượng tinh trùng, tinh tử, tế bào mầm sinh tinh; hạn chế phân mảnh tế bào mầm; bảo vệ ADN tinh trùng: trên quang trường hầu như không có dấu hiệu phân mảnh ADN.

4. Mô hình phóng xạ ⁶⁰Co (7 Gy): Sau 7 ngày, phác đồ kết hợp viên nang KS (1 viên/kg và 2 viên/kg) với testosteron (1 mg/kg) làm tăng nồng độ testosteron, giảm FSH, LH; tăng trọng lượng tinh hoàn, túi tinh - tuyến tiền liệt, cơ nâng hậu môn, tăng thể trọng; tăng số lượng tế bào Sertoli, tế bào Leydig, tinh trùng; tỷ lệ ADN phân mảnh khoảng 5% (lô bệnh lý khoảng 95%).

    - Kết luận: Trên các mô hình cắt 2 tinh hoàn, natri valproat, ⁶⁰Co, phác đồ kết hợp viên nang KS (1 viên – 2 viên/kg) với testosteron thể hiện tác dụng tăng testosteron và các cơ quan sinh dục phụ, cũng như các chỉ số tinh trùng. Phác đồ kết hợp viên nang KS với testosteron an toàn trong thời gian 60 ngày.

SUMMARY OF NEW FINDINGS

Background: Male hypogonadism tends to increase and more common in old age. The trend of choosing herbs or traditional remedies to support the treatment of male hypogonadism is receiving more and more attention.

Objectives and Methods: Capsule KS was packed in red capsule, No.0 which contains 540 mg of herbal extracts, including: Herba epimedii 50%, Allium tuberosum 20%, Polyscias fruticosa 20% and Ginkgo biloba 10%. Experimental study was established on mice, before and after intervention with small population.

Results:

1. Evaluation of safety: After 14 days, the combination regimen of capsule KS (1 capsule/kg) with testosterone (1 mg/kg) didn’t change the concentration of testosterone, total protein, body weight, weight of seminal vesicles - prostate, testicles, levator ani muscles of normal male mice compared with control group drinking distilled water. After 60 days, the combination regimen (KS + testosterone) increased the concentration of testosterone, total protein, weight of seminal vesicles - prostate, testicles, levator ani muscles.

      2. Two testicles removal model: After 15 days, the combination regimen of capsule KS (1 capsule/kg) with testosterone (1mg/kg and 2mg/kg) showed the effect of increasing testosterone levels, total protein, weight of seminal vesicles - prostate, levator ani muscles, compared to the removal group and asymptotical to the normal group.

3. Sodium valproate 500 mg/kg model: After 35 days, the combination regimen of capsule KS (1 capsule/kg and 2 capsules/kg) with testosterone (1mg/kg) increased testosterone levels, weight of testicles, seminal vesicles - prostate, levator ani muscle compared to the sodium valproate group. Combination regimens had the effect of protecting testicular tissue: improving seminiferous tubule morphology; increasing number of Sertoli cells, Leydig cells, number of sperm, spermatozoa, spermatogenic germ cells; limiting fragmentation of germ cells; protecting sperm’s DNA with almost no sign of DNA fragment under microscopic optical field.

       4. ⁶⁰Co radiation (7 Gy) model: After 7 days, the combination regimen of capsule KS (1 capsule/kg and 2 capsules/kg) with testosterone (1mg/kg) increased testosterone levels, weight of testicles, seminal vesicles - prostate, levator ani muscle, body weight, number of Sertoli cells, Leydig cells, sperm; decreased FSH, LH. DNA fragmentation rate was about 5% compared to radiation group of 95%.

Conclusion: In two testicular removal, sodium valproate, ⁶⁰Co models, the combination regimens of capsule KS (1 capsule – 2 capsules/kg) with testosterone showed increment of testosterone and accessory sexual organs, and other sperm indicators. The regimen of combining capsule KS with testosterone was safe for 60 days.
