Đặt vấn đề: Nghiên cứu kết quả tán sỏi thận qua da đường hầm nhỏ với tư thế nghiêng cải biên có hướng dẫn siêu âm

Đối tượng và phương pháp nghiên cứu: Các TH có sỏi thận được tiến hành TSTQD với tư thế nghiêng cải biên dưới hướng dẫn của SA tại khoa Phẫu Thuật Điều Trị Sỏi Thận Chuyên Sâu, Bệnh viện Bình Dân từ tháng 02/2020 đến tháng 09/2022. Nghiên cứu tiến cứu can thiệp không nhóm chứng

Kết quả: Tỷ lệ chọc dò vào hệ thống đài bể thận thành công là 100%. Hiệu quả với tỷ lệ sạch sỏi: sạch sỏi ngay sau mổ 88,6%, sạch sỏi sau 1 tháng 91,4%, sạch sỏi sau 2 tháng: 94,3%. Tỷ lệ tai biến − biến chứng: clavien 1: 4,29% (2 TH chảy máu + 1 TH sốt), clavien 2: 7,14% (5 TH sốt cần điều trị nâng bậc kháng sinh), clavien 3a: 1,43% (1 TH nhiễm khuẩn huyết). Tỷ lệ nội soi niệu đạo − bàng quang ngược chiều: Đặt thông niệu quản 18/70 TH (25,7%), Đặt double J 5/70 TH (7,1%), tán sỏi niệu quản kèm theo 5/70 TH (7,1%).

Kết luận: Tán sỏi thận qua da đường hầm nhỏ với tư thế nghiêng cải biên có hướng dẫn siêu âm là an toàn, hiệu quả với tỷ lệ biến chứng thấp.

SUMMARY OF NEW FINDINGS

Background: This study was to evaluate the safety and efficacy of ultrasound-guided mini percutaneous nephrolithotomy (US-guided miniPCNL) in a modified lateral position with extended legs for kidney stones

Objectives and Methods: This prospective study included 70 patients underwent miniPCNL with US-guided renal access in the modified lateral position with extended legs from 02/2020 to 09/2022. The outcomes included the rate of successful access, complete stone clearance, operative characteristics, postoperative complications, and the need for an additional procedure.

Results: Successful renal access rate was 100%. Complete stone clearance rate: Stone-free rate immediately after surgery 88.6%, stone-free rate after 1 month 91.4%, stone-free rate after 2 months 94.3%. Rate of complications: Clavien 1: 4.29% (2 cases of bleeding + 1 case of fever), Clavien 2: 7.14% (5 cases of fever requiring upgraded antibiotic treatment), Clavien 3a: 1 .43% (1 case of sepsis). Rate of retrograde urethro - cystoscopy: ureteral catheterization 18/70 TH (25.7%), Double J insertion 5/70 TH (7.1%), ureteral lithotripsy with 5/70 (7.1%)

Conclusion: mini percutaneous nephrolithotomy (US-guided miniPCNL) in a modified lateral position with extended legs for kidney stones is safe and effective with a low complication rate
